GUIDELINES FOR THE SURVEY:

Answer all questions by rating the statement as noted:

1 - don’t care/not started….5 - very important/mastered

Rate the Order of Importance FIRST, then re-read each statement and rate the Family Achievement

SECOND.

There are no right or wrong answers.

If you do not understand a statement or if it does not apply to you - leave it blank.

Mark the answer to each statement based on first response/answer that comes to mind.

Your answers to questions are not set in stone – you may decide to change them later in the process,

and that’s okay.

Do not talk with your spouse or other family members while answering the survey.The following is a quick assessment to determine what areas are most important to you and your

family and where you feel you currently are in each of these areas. This will help identify your

priorities as it relates to the different aspects of WEALTH.
